Generations Day Release Form

*For Parents' Use Only*
If your special guest will be taking your child home at the end of Generations' Day at 12:30pm on December 22nd, you must complete the dismissal form by December 8th. To do so:
  1. Log into your Lions Log In account by clicking the "Lions Log In" button at the top of vbsds.org
  2. Then click here to be taken to your forms page, OR click the link in the yellow bar along the top of the page (see screen shot)
  3. Please fill out the "2023 Generations' Day Permit For Student Release" for each current student.
Reminder - there is no Kids Club extended care after the 12:30pm dismissal.
